New patients attending Rochedale Family Practice for the first time should complete the registration form before their appointment where possible. Parents or guardians can complete the form for children or dependants.
What should I bring to my first appointment?
Please bring your Medicare card, any concession cards you use, photo identification, a current medication list, and any relevant specialist letters, imaging, or recent results that may help your doctor understand your care.
